Self Regional Healthcare will be at Emerald High School on Thursday, April 27, 3:30-5:30 p.m. for sports physicals.
The physicals are available for any Greenwood School District 50 student.
Please note:

Cost is $25.

Sports physicals by Self Regional Healthcare will be good for any sport played during the 2023-2024 school year.

Student must be checked in and paid by 5:30 p.m.

The Physical Form must be completed prior to the physical and signed by a parent or guardian.

Only cash or check will be accepted as payment. If writing a check, please make it payable to Self Regional Healthcare. Questions? Contact the EHS Athletic Department.
